You have the following rights with respect to your protected health information. To exercise any of these rights, please submit a written request to our office.
- Right to Inspect and Copy You have the right to inspect and obtain a copy of the health information we use to make decisions about your care, including your medical records. We may charge a reasonable fee for providing copies.
- Right to Request Amendment If you believe health information we have about you is incorrect or incomplete, you may ask us to amend it. We may deny your request if the information was not created by us or if we determine the information is accurate and complete.
- Right to an Accounting of Disclosures You have the right to request a list of disclosures we have made of your health information, other than for treatment, payment, healthcare operations, or disclosures you authorized.
- Right to Request Restrictions You have the right to request restrictions on how we use or disclose your health information for treatment, payment, or healthcare operations. We are not required to agree to your request, but if we do agree, we are bound by our agreement.
- Right to Request Confidential Communications You have the right to request that we communicate with you about your health information by alternative means or at an alternative location — for example, requesting that we contact you only at a specific phone number.
- Right to File a Complaint If you believe your privacy rights have been violated, you may file a complaint with our practice or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
Uses Requiring Your Written Authorization
Other uses and disclosures of your health information not covered by this Notice, or not permitted by applicable law, will be made only with your written authorization. You may revoke any written authorization at any time by submitting a written revocation to our office. Revocation does not affect disclosures already made in reliance on your authorization.
